Summer Camp Medical Form Information

Connecticut Rivers Council has revised its medical form procedures year in order to comply with CT State Youth Camp regulations and to clarify what is required of camp attendees and adults. This document lists some of the reasons for these changes

Summer Camp Medical Form

VERY IMPORTANT!! Scouts and Leaders will not be allowed to stay in camp without a completed Medical Form.

Scouts and adult leaders under 40 yrs of age must have a physical exam by licensed physician conducted after 7/12/2006 (36 months)

Leaders age 40 or over must have a physical exam by licensed physician conducted after 7/12/2008. (past 12 months)

Completed form due to Mike Patrie prior to June 30.

Summer Camp Medication Form

This is a medical form that is required when Parents/guardians

are requesting medication administration to their child from camp staff.

Completed form due to Mike Patrie prior to June 30.

Summer Camp Allergy Treatment Form

This is a medical form is required when the scout has as serious allergy. This form lists the allergy treatment plan and give permission for the administration of medications by camp personnel.

Completed form due to Mike Patrie prior to June 30.

Family Barbecue, Car Pool, Troop Photo Form.

2009 Family Barbecue / Car Pool / Troop Photo Form. Fill this form out if you are attending the Family Barbecue on the July 17, if you can drive scouts to and/or from camp, if you would like to purchase a Troop photo.

Completed form due to Mike Patrie prior to July 12.
