Medical Class 1 & 2
This is the medical form that is required for all scouts and their leaders under age 40 who are attending summer camp this year. This include a physical exam within 3 years of July 13th. (Please make sure that immunization dates are noted on all health forms. It is not acceptable under State of Maine camp rules and regulations to put 'up to date.')

Completed medical form due to Mike Patrie prior to June 29.

Medical Class 3
This is the medical form that is required for all leaders 40 years of age or older who are attending summer camp this year. This include a physical exam within 1 year of July 13th.

(Please make sure that immunization dates are noted on all health forms. It is not acceptable under State of Maine camp rules and regulations to put 'up to date.')

Camp Emergency Medication Plan
This is a medical form that is required for scouts and leaders who have asthma and require inhalers and/or Epi-pens for treatment.

Photo Release Form
This is photo release form allowing the Pine Tree Council to keeps a library of photographs, slides, and videos to use for future promotional pieces. These promotional pieces may include camp brochures, videos, Pine Spills publications, web sites, and slide shows.

Completed form due to Mike Patrie prior to July 1.

Family Barbecue, Car Pool, Troop Photo Form.
2008 Family Barbecue / Car Pool / Troop Photo Form. Fill this form out if you are attending the Family Barbecue on the July 18, if you can drives scout to and/or from camp, if you would like to purchase a Troop photo.
